University of Nebraska–Lincoln Glenn Korff School of Music (GKSOM) Spring Indoor Percussion Instrument Rental Agreement

By submitting this form, I understand I will be assigned a University of Nebraska–Lincoln Glenn Korff School of Music (GKSOM) instrument. I acknowledge that by submitting this form I agree to the following conditions:

I understand the non-refundable $20.00 per semester rental fee will be applied to my student account and will appear on my UNL billing statement.

I understand that the date this instrument is due to be checked in Friday, May 1, 2026. 

This instrument is provided in support of my work at the GKSOM. THIS INSTRUMENT WILL NOT BE USED FOR NON-UNIVERSITY EMPLOYMENT, PERFORMANCES, AUDITIONS, OR ACTIVITIES AND IS TO BE USED FOR THE INDOOR PERCUSSION ENSEMBLE REHEARSALS, EVENTS, AND APPROVED PERFORMANCES ONLY. Violation of this policy may lead to a suspension of the use of the instrument for the remainder of the semester. Exceptions to this policy may be granted in writing by the Director of Bands in consultation with the Band Faculty. I understand that individual faculty or staff members are not empowered to grant this permission.

The University of Nebraska–Lincoln GKSOM maintains full ownership and control of this instrument and use thereof. Under no circumstances may it be loaned to another person or institution. I agree to be solely responsible for this instrument and all issued accessories. included with this instrument. I will immediately report any damage to the GKSOM and will return the instrument for needed repairs.

I understand if I do not return the instrument by the due date and according to instructions as explained above, I will be fined as follows:

• 1–7 days past due: $25.00 (non-refundable) with a hold placed on my records/registration.

• 8–14 days past due: $50.00 (non-refundable).

• 15 days past due: The instrument will be reported to University Police as stolen GKSOM property.

I agree to pay the current replacement cost of this instrument to the GKSOM in the event it is lost, stolen, or damaged beyond repair. I am aware that the University does not maintain insurance on the instrument. Furthermore, I agree to pay for all missing accessories, and damage not considered normal wear, as determined by a qualified instrument repair technician retained by the GKSOM.

If for any reason I leave THE INDOOR PERCUSSION ENSEMBLE before the end of the semester, I will immediately return the instrument to the UNL Percussion Faculty.
